Please acknowledge the terms of the San Francisco Symphony Community of Music Makers Liability Release and Media Waiver: I AM AT LEAST 18 YEARS OF AGE, HAVE READ, UNDERSTAND AND AGREE TO ALL TERMS AND CONDITIONS CONTAINED HEREIN, INCLUDING AGREEING TO THE RULES, REGULATIONS AND GUIDELINES OF THE SAN FRANCISCO SYMPHONY’S COMMUNITY OF MUSIC MAKERS PROGRAM. I AGREE TO THE FOLLOWING RELEASE OF LIABILITY AND MEDIA WAIVER: In consideration for participation in the San Francisco Symphony’s Community of Music Makers program, participant agrees to assume all risk of personal injury, loss or damage to his/her person or property. San Francisco Symphony may revoke the privilege to participate in this adult education program for reasons of misconduct, nuisance, or similar behavior by participant. The participant agrees to hold harmless and indemnify the San Francisco Symphony and its directors, officers, members of the board of governors, employees, agents and volunteers from any and all claims, actions, injuries or damages which may be sustained or caused by participation in or connection with these activities. I hereby give my consent to grant the San Francisco Symphony (“SFS”), or any entity approved by the SFS, to take photographs, motion pictures, digital images, sound recordings, and/or DVD/video tapes of me, singly or in a group, for the purpose of marketing, public relations, documentary production, broadcasts, and informing constituents and the general public of the activities of the Community of Music Makers program. SFS may publicize the Program using clips of all or part of the Interview via television and radio (including commercials and promos), the Internet (audio-visual and audio-only), home video products (in any format now known or hereafter devised, including but not limited to as bonus material in a home video product) and promotional material sent as a streaming email montage to promote the Program. SFS shall be under no obligation to actually use Subject’s name, voice likeness, and/or biographical information. I grant SFS, or any entity approved by the SFS, permission to interview me and to use my interview or likeness in any and all of its publications and in any and all other media, whether now known or hereafter existing, controlled by SFS for news and promotional purposes related to Community of Music Makers and all initiatives of the SFS. I agree that the rights granted hereunder shall include the perpetual, worldwide right of SFS to edit, telecast, cablecast, webcast, rerun, record, publish, reproduce, use, license, print, distribute or otherwise exploit, in any manner and in any medium or forum whether now known or hereafter devised in whole or in part. Without limitation, I will make no monetary or other claim against SFS for the use of the interview or the broadcast of my voice and/or the publication or use of photographs, motion pictures, digital images, sound recordings, and/or DVD/video. This release is for San Francisco Symphony use only and does not grant permission for program participants to use photographs or recorded media of other participants, San Francisco Symphony events, concerts, employees or concert hall property, whether for semi-public/public consumption or personal gain. Participants are responsible for their comments and information posted to the World Wide Web, as provided by law. All film or digital files created are the sole property of the San Francisco Symphony and may not be used without written consent of an authorized representative of the SFS.
